Print this custom skirt order form & fill it out and send it along with your payment and your tshirts to the address on the form.  Once the order is received, your shirts will be combined with our additional fabric options and we will be in touch to get your opinions. Once the details are worked out, creation will begin. Turn around time is approximately 2 weeks.

And by the way, there is no extra charge for creating this one of a kind wearable art piece for you;  it brings me great joy to give new life to your special t-shirts!
